Steward’s health care model integrates physicians and caregivers to provide high quality, efficient care for patients throughout the health care continuum. This integration improves wellness and lowers health care costs through better management of chronic diseases, reduces medical errors and eliminates duplicative and unnecessary testing.
The Steward Community Health plan is a limited network plan providing benefits primarily through the Steward Health Care Network (SHCN). SHCN is a fully integrated community care organization comprised of 11 hospitals, over 2,000 physicians, and a full service network of ancillary providers including hospice and a home care agency. In addition, SHCN has contracted with Massachusetts General Hospital and Brigham & Women’s Hospital for services not available within the Steward Health Care Network.
Pharmacy benefits are provided through MedImpact Healthcare Systems, Inc. and include a broad network of participating pharmacies including CVS, Rite Aid, Walgreens, and many others.
You must choose a Primary Care provider from the Steward Health Care Network of providers. For pediatric members (children under 19) you may choose a PCP from either the SHCN or the Affiliated Pediatric Practice, a Massachusetts General Hospital network.
Services received without a referral may not be covered; you may be responsible for full payment, which will not be credited to your out of pocket limit.
Services received from Tufts Health Plan providers are covered at 100 percent if referred by your PCP and authorized by a Steward Referral Coordinator.
