Medicare Shared Savings Program

Steward National Care Network, Inc. is a Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO). If your primary care provider is participating in this Steward Program, you will receive a letter welcoming you to the Medicare Shared Savings Program. As MSSP ACO beneficiaries, your provider and specialists will work together to:

Develop a treatment plan specific to your health care needs
Coordinate the care you receive
Deliver care more effectively to keep you well and improve your overall health and quality of life

Your provider’s participation in the MSSP does not change your Medicare benefits and it does not limit the doctors you can choose to see. Every benefit you currently have under Medicare remains the same. MSSP gives your provider the tools and resources they need to take better care of you.

Please note, the MSSP ACO is only for Fee-For-Service (FFS) Medicare beneficiaries and not Medicare Advantage based beneficiaries.

Our community hospitals and providers believe that having access to world class health care, in the communities where you live and work, is the solution to the health care cost issue. By working together to provide care that meets your specific needs, our providers can work to improve your quality of life and make a difference in your overall health.

How does it work?

Under the MSSP, Medicare will give your health care team the information they need to create a wellness program specifically for you. Your health care providers will work closely with you to create treatment plans for your overall health. They will care for you when you are sick and work closely together to keep you well.

What will be different for you?

To keep you well, your health care providers need to get in touch with you more often. This means that you will likely have more conversations in person, by phone and email to see how you are doing. You may be contacted to discuss important tests, appointments, and screenings that you may need.
